Health Care Plans. As a managing member, I was eligible to participate in the firm's benefit plans. These benefits include medical insurance (including vision coverage), dental insurance, and HSA. The premiums of the medical, vision, and dental plans were paid by Spark Capital. I was provided short-term and long-term life insurance and disability insurance, both of which had premiums that were covered by Spark Capital. As a former managing member, I am still eligible to participate in these plans. If I choose to participate in these plans, Spark Capital will make no further payments and I will pay the full amount of the premiums.

I will no longer receive this benefit upon confirmation, but may pay Spark Capital to retain the services of my current secretary for personal non-official purposes.
